Write tool descriptions that say when to use the tool

Descriptions under 20 characters usually restate the name. Add when to use it and what it returns.

356 of 15,575 reachable MCP servers (2.3%) have this issue today.

Why it matters

"Get weather" does not tell a model whether the tool does forecasts, history or the current hour. One extra sentence removes most wrong-tool calls.
